Description

Communicare Oxford MS seeks a compassionate Direct Care Worker to support adults and youth with behavioral health and intellectual/developmental disabilities in community and home settings. In this mission-driven role, you will assist with daily living tasks, promote independence, reinforce treatment and recovery goals, and provide safe, trauma-informed support. Responsibilities include monitoring and documenting client progress, assisting with transportation, engaging clients in community activities, and collaborating with a multidisciplinary team. Ideal candidates are reliable, patient, team-oriented, and committed to improving wellness across North Mississippi communities.
Provide hands-on support with daily living, hygiene, and household tasks for adults and youth with behavioral health and IDD needs.
Support client treatment and recovery goals by following care plans and reinforcing therapeutic skills in home and community settings.
Monitor client safety and behavior, report changes promptly, and document services accurately and timely.
Assist with transportation to appointments, programs, and community activities to promote independence and inclusion.
Collaborate with clinical and community teams to ensure client-centered, trauma-informed, and culturally responsive care.
